Personal care services enhance their mental and physical well-being, helping them to feel positive about their appearance. We provide assistance with bathing, dressing, grooming, and bathroom care. Community skills such as transportation within the community, shopping for personal care items, food, clothing, and navigating the individual’s community settings. A Shared Living provider is not able to deny the individual contact or visits with family for any reason, including as a consequence for challenging behaviors. Parameters around family visits (phone calls, supervised/unsupervised, location, etc.) are determined by the individual’s ISP team to include the service coordinator, case manager, guardian, and/or the judge assigned to the case. This is typically determined by the individual’s team and communicated to the Shared Living provider.

We interviewed a few agencies, but Extended Family immediately had the right feel for us. Care Coordinator
If you feel called to serve those with disabilities, becoming a Shared Living Provider will allow you to form empowering relationships and provide support in a transformative way. We do ask that you do your best to keep non-emergent calls to Shared Living Specialists limited to regular business hours Monday to Friday from 9 am to 5 pm. Home Care
Due to the COVID-19 pandemic, a majority of our training classes are virtual. In most cases, the Shared Living provider is responsible for transporting the individual to appointments, school/vocational programs, and any other activities outside of the home. While we understand that things may come up, please contact your Shared Living Specialist if you are not able to provide transportation to a necessary appointment, school/vocational program, etc. due to a special circumstance.
ACN offers a shared living option, previously known as extended family homes, in which a person with autism or other developmental disability makes his/her home with an individual or family not related to him/her. This shared living model provides the advantage of close relationship while maintaining the security of round the clock staffing and supervision. ACN currently operates shared living arrangements in Omaha, Lincoln, Wahoo, Fremont, and Valley.

The individual pays room and board directly to the Sub Care provider. I have worked with several home care companies and Extended Family in Denver was the best by far. They would constantly go out of their way to help my sister with the many issues that she had. They were very flexible and would do last minute things that came up. I was always able to contact them by phone, email and text and they would quickly respond. This payment accounts for their portion of the living space, food, and household utilities. If the individual requires or desires additional utilities (i.e., an extra tier of cable) their room and board can be adjusted when the team is in agreement. The room and board fees do not apply to individuals that are contracted through a letter of agreement with Probation, Child and Family Services, or Saint Francis Ministries. Per KVC policy, Shared Living subcontractors are not allowed to subcontract with other agencies and/or provide Shared Living services to individuals not receiving oversight with KVC. Until the COVID19 restrictions, I was able to help and visit my mom on most days that Susan wasn't there. However, once restrictions were in place, Susan was my mom's only help. She became my connection with my mom, letting me know how she was doing and what groceries/supplies she needed. The care Susan provided for my mom was excellent, but she also helped me, by being there when I couldn't.

If an individual is allowed alone time in the community, or for extended periods of time, it is noted in the Individual Support Plan created by the client’s Services Coordinator.
